New Patient Registration
Patients who wish to register with the practice will be asked to complete the practice registration forms and provide photographic ID and proof of address.
Once we are in receipt of signed documentation from new patients, we will create a patient account on the computer and send an electronic application to the Business Service Organisation. They generally respond with 15 working days changing the "applied" status of the registration to "permanent".
Patients applying to register who are not UK residents or patients who are returning to live in the UK will need to complete an HS22X form which will require more detailed information in order to complete the registration process.
It is important that everyone registering provides a list of current repeat medication from the previous GP when returning your completed registration forms.
You will be given a copy of the practice leaflet with your registration pack. All practice information is also detailed on this website.
Please note it can take up to 6 weeks before we receive previous medical records.
