Self Refer Services

Patients can now self refer for Physiotherapy, Podiatry and some Occupational Therapy services including Hearing.

Physiotherapy - Patients over 16 years old can now self refer to Physiotherapy if suffering from low back pain, neck pain or recent injuries such as strains and sprains, or join and muscular pain. For all other conditions you should consult with your GP. (service commenced on 15 June 15).

How to self refer:

2) Download attached form https://setrust.hscni.net/physio-form/

3) Collect a hard copy form from GP Reception

Podiatry - e.g. arch/heel/corn/bunion pain and other medical conditions that affect the feet. Patients can self refer to Bangor Hospital Podiatry Team by completing the attached form and posting to:

Bangor Community Hospital, Castle Street, Bangor BT20 4TA.  GP appointment is no longer required.

Occupational Therapy - e.g. toileting equipment, ramps, bathing/shower/wheelchairs etc. Note not all adaptations requested by Patients are recommended by OT.

Patients can self refer by telephoning Bangor Occupational Therapy Team directly on 02891515268 or 02891515351 (Mon to Fri 9am to 5pm). OT will make contact with the GP if they require additional information.
